Here is LSMW Exemple which uses the folowing structures :
IBIPTLST - Header
IBILTLOP - Operations
IBIPTMAT - Materials
IBIPMPAC - Maint Pack.
IBIPTEXT - operations long text
step 1 : prepare this source CSV files 
Step 2 : Store the files at C:\temp
Step 3 : import the attached LSMW project
This is the exemple LSMW project
Option 2 : Using the IBIPTool - Which I just text it with the above mention data
Step 1 :Create template
3. Upload the template from Excel SAP connectivity (using RFC) - Requires office 32bit
4. Generate Flat file and pload in IBIP Transaction ( relvant for background batch or office 64 bit
